Medical Care Manager

Use this tool to gather all medical information in one place. You can create lists of information, people and resources that you need on a regular basis or that can be easily accessed by other caregivers. Type in the box and then click on “Add a ….” to save the information in the list to the right.

  • Personal Medical information: Use this area to keep track of important information that you may need for other caregivers or doctors.
  • Conditions and Providers:  Enter your loved ones conditions and information about providers that care for them. This information will be helpful for family and other caregivers.
  • Medications:  This area will help you keep track of medications and their schedule.
  • Tests & Treatments :  Use this area to record the results of tests and to make note of treatments and dates.
